Is a subscription required?

That is the common shape. A paid-up licence can also be written. It depends on who pays, and when.

Is a web application SaaS?

Not if it is private to one company. Yes if many customers sign in to their own accounts.

SaaS is software opened to many customers. It is not the same thing as a panel built only for you.
